Because we are a true medical facility, owned and operated by Dr Jennessa Iannitelli, we require all patients to complete all patient forms and medical history before their treatment. By gaining a comprehensive understanding of your medical history and individual concerns, we can ensure a personalized, safe and more effective beauty and wellness experience. Please complete this pre-registration before coming to the office for your appointment. That’s OK!

Why do you need my credit card information?
To ensure seamless scheduling and the ability to provide the highest level of service to all of our clients, we ask for credit card information at the time of booking. This deposit guarantees your reservation and is fully applied to your service cost. This policy supports our 48 hour cancellation guidelines safeguarding both your time and that of our dedicated team. We require $99 deposit to hold your appointment for services with our aestheticians and $150 for appointments with Dr. Iannitelli, the total deposit will be applied to your service.
We respect your time and commitments, and in the event that plans change, we are ready to assist with rescheduling your appointment if notified at least 48 hours in advance. Appointments that are missed without 48 hours notice, will result in a fee of $99 for appointments scheduled with our aestheticians and a fee of $150 for appointments with Dr Iannitelli.
Rest assured, if cancellations are made within the specified time frame, your deposit will be fully refunded. Our aim is to provide a fair and courteous policy for all clients, which includes a fee for missed appointments or late cancellations to accommodate the dedicated time set aside for you.
